LIABILITY RELEASE/CONSENT AND INFORMATION RELEASE
I hereby grant the Beer Store rights to the free use of my name, image, likeness, and sound of my voice as recorded on audio and/or video, in any broadcast, telecast, print account, social media platform or any other account in any medium without payment or any other consideration. I understand that my image may be edited, copied, exhibited, published or distributed and waive the right to inspect or approve the finished product where it involves the use of my likeness.
I hereby release and hold harmless the Beer Store from any and all liabilities related to the use of my name, image/picture and/or video (the “Personal Release”). I understand that this Personal Release is perpetual in time and that it encompasses, without limitation, any copyright or right of publicity or privacy that I may have in my name, picture, and/or video. Additionally, I acknowledge that there is no geographical limitation on where these materials may be distributed.
By signing this release, I understand this permission signifies that photographic or video recordings of me may be electronically displayed via the Internet.
In signing this form, I acknowledge that I have completely read and fully understand the above release and agree to be bound thereby.
